A network of more than 10 navigators created many resources and tools; however, the constant flow of information led to information gaps and inefficiencies. To address this problem, a comprehensive overview of the Transition Specialist role and the creation of the Transition Specialist Resource Hub promoted a common understanding of the function within and across member agencies.
Transition Specialists are employed at each of SBCAE’s member organizations. This network of 10+ navigators is charged with implementation of the consortium’s No Wrong Door model, as well as supporting student transitions to further education, the workforce or community engagement. Over the years, many resources and tools have been developed by Transition Specialists, and there is a constant flow of information among the group to stay informed about new programs, courses or community resources. Having that information spread out across different agencies and many people’s inboxes led to information gaps and inefficiency.
The SBCAE Transition Specialist team developed a ‘one-stop’ portal for resource and information sharing, called the “TS Resource Hub”. Starting with an in depth review of the function of the TS, the team created a website for all TS to access and share resources in the following content areas: intake/assessment, transition to education, transition to career, transition to community, outreach, support services and data. The site is also available to other staff in the consortium.
A comprehensive overview of the Transition Specialist role, promoting a common understanding of the function within and across member agencies. A single access point for resources and tools transitions specialists need on a regular basis.
Transition Specialists report a better understanding of their role, and greater efficiency in information sharing with their peers. The TS Resource Hub also functions as an onboarding tool for new Transition Specialists.
